As part of our Community Health Needs Assessment (CHNA), Cook Children's Center for Community Health is dedicated to preventing injury and illness, and embracing wellness. Knowing every child's life is sacred, we promise to improve the well-being of every child in our care and our communities by gathering valuable insights from parents, caregivers and community leaders across our eight county service region – Collin, Denton, Grayson, Hood, Johnson, Parker, Tarrant and Wise counties. Below, you will find detailed information about our leader and parent/caregiver surveys and a link to our interactive data dashboard.
Leader survey: ETC Institute administered a Community Health Needs Assessment Leader Survey for Cook Children's and the Cook Children's Center for Community Health. The purpose of the survey was to gather information from leaders about children's health issues in our service region. The data collected from the survey will help the CHNA research team better understand the concerns and issues community leaders have regarding children's health.
Parent survey: ETC Institute administers a Community Health Needs Assessment Survey for Cook Children's Health Care System. The purpose of the survey is to gather information from parents of children ages 0-17 years living in Cook Children's service region. The data collected from the survey will help Cook Children's and the community better understand the current state of children's health and better focus on improving the health and wellbeing of children in the region.
